How do I know if my hair transplant failed?

First, check the date. A transplant cannot be judged early. Transplanted hairs shed between weeks 2 and 8, new growth starts around months 3 to 4, and the final result takes 12 to 18 months. After 12 months, these are the common signs that a result needs a second look:

  • Pluggy or "doll hair" look: visible clumps of hair at the hairline instead of a soft, irregular edge.
  • Wrong angles: hairs that grow straight up or in the wrong direction, so they cannot be styled and stick out.
  • Low density: you can see the scalp through the transplanted area even with hair fully grown.
  • Unnatural hairline: too low, too straight, too round, or not matched to your age and face.
  • Over-harvested donor area: the back of the head looks thin, patchy or "moth-eaten".
  • Visible scarring: a wide strip scar (from older FUT surgery) or white dots in the donor area.
  • Poor growth: clearly less hair than the number of grafts you were told you received.

Why should I wait 12 months before a second hair transplant?

There are three reasons.

  • Growth is still happening. Density keeps improving until month 12 and sometimes month 18. A result that looks thin at month 6 may look very different a year later. See hair transplant results after one year.
  • The scalp needs to heal. Operating on tissue that has not settled can harm grafts that are still growing.
  • You need a clear map. The doctor can only plan a repair once it is clear which grafts survived and where the gaps are.

The exception is an urgent medical problem, such as infection or pain that gets worse. That needs a doctor straight away, not in 12 months.

What can be fixed in a failed hair transplant, and what cannot?

Being honest about this is the most important part of a repair consultation. Some problems have good solutions. Others can only be improved or camouflaged.

The hard limit is simple: every follicle taken from the donor area is gone from there for good. A repair cannot create new donor hair. It can only use the reserve you still have more wisely.

What is the donor reserve, and why does it matter for repair?

Your donor reserve is the number of permanent follicles at the back and sides of the head that can still be harvested safely. These follicles resist the hormone that causes male pattern hair loss, which is why transplanted hair keeps growing.

In a first procedure, a good plan leaves enough reserve for the future, because hair loss often continues over the years. In a repair case, the first job is to measure what is left. If an earlier clinic took too many grafts, or took them unevenly, the reserve may be small. Then the plan has to focus on the zones that matter most for your face, usually the hairline and the front.

Why do patients come back after cheap hair transplant packages abroad?

We regularly see patients in Lebanon who had their first procedure abroad. Many good surgeons work in every country, so the issue is rarely the destination itself. It is usually how the package was set up. Common patterns include:

  • A very high graft count in one day, sometimes more than the donor area could safely give.
  • Little time with the doctor, with most steps done by a team the patient never met at the consultation.
  • A one-size hairline that does not match the patient's age or face.
  • No follow-up once the patient flies home, so problems are noticed late.

How does a hair transplant repair consultation work?

  1. Bring your history. The date of the first surgery, the technique (FUE, DHI or FUT), the number of grafts you were told, and any photos from before and after.
  2. Scalp and donor examination. Dr. Hala Ashkar checks graft angles, density, the hairline, scars, and how much donor reserve remains.
  3. Check for ongoing hair loss. If your own hair is still thinning, that has to be treated too, or a repair will look patchy again in a few years.
  4. An honest plan. What can be fixed, what can only be improved, how many sessions it may take, and whether surgery is the right step at all.
  5. A clear quote. Repair is priced after the examination, because it depends on the number of grafts and the technique needed.

Which technique is used for a second hair transplant?

In many repair cases, grafts are harvested with FUE and placed either into FUE channels or with DHI using the Choi pen.
