Is a Hair Transplant Painful? Anesthesia & Pain in Lebanon 2026
Is a hair transplant painful? What you feel during local anesthesia, extraction and implantation, the first nights after, and how pain is managed in Lebanon.
Is a hair transplant painful? For most patients, no. A hair transplant in Lebanon at Healthy Beauty Clinic is done under local anesthesia, so the only real discomfort is the first few small injections that numb the scalp. After that you feel pressure and movement, not pain. The first nights bring mild soreness that prescribed painkillers usually control.
What does a hair transplant feel like, step by step?
Most of the fear comes from not knowing what happens in the chair. Here is what patients typically feel at each stage of the day. Your own experience depends on your pain threshold, the size of the session and your doctor's plan.
| Stage | What you usually feel | How long it lasts |
|---|---|---|
| Local anesthesia injections | Small stings and a burning feeling for a few seconds per area | The first minutes of each area |
| Extraction (donor area) | Pressure and light tapping, no sharp pain once numb | The first part of the session |
| Implantation (recipient area) | Light pressure on the scalp, sometimes a faint pulling feeling | The second part of the session |
| When the numbness wears off | Tightness and soreness, mostly in the donor area | Mainly the first 2 to 3 days |
| The first week | Mild tenderness, itching as scabs form, possible forehead swelling | Fades day by day |
For the full timing of the day itself, read our procedure day step-by-step guide.
Does the local anesthesia injection hurt?
Yes, a little. This is the part most patients remember. The doctor numbs the donor area at the back of the head and then the area being treated, using several small injections.
Each injection feels like a short sting or burning sensation. It fades within seconds as the anesthetic takes effect. Once an area is numb, it usually stays numb while the doctor works there, and it can be topped up if needed.
Patients often describe it as more uncomfortable than painful, similar to a dental injection. If a spot starts to feel sensitive during the session, tell the team straight away.
Will I be asleep?
No. At Healthy Beauty Clinic the procedure is done under local anesthesia, which is included in every package. You stay awake and can talk, watch something, listen to music and take breaks. If you are very anxious, say so at the consultation, so the doctor can tell you what options suit your case.
Is extraction painful?
Not once the donor area is numb. During extraction, individual grafts are removed one by one from the back and sides of the head with a very fine punch.
What you notice is pressure, slight tapping and the sound of the instruments. You usually lie face down or on your side for this part, so the main complaint is staying still, not pain. Short breaks help.
Is implantation painful?
No. Implantation is often the easiest part of the day. The grafts are placed into the numbed recipient area, and most patients feel only light pressure. Many sit up, chat or watch a film while it happens.
Is DHI less painful than FUE?
The difference is small. Both techniques use the same local anesthesia and the same way of taking grafts from the donor area. Because extraction is identical, the donor area feels the same afterwards with either method.
- FUE: the doctor first opens tiny channels in the recipient area, then places the grafts into them.
- DHI: each graft is loaded into a Choi pen and implanted directly, without opening channels in advance.
- What that means for comfort: some patients find DHI a little gentler on the recipient area, but both are done under full local anesthesia, so neither should hurt during the procedure.
- No-shave option: DHI can be done without shaving the whole head. That changes how you look after surgery, not how much it hurts.
The doctor chooses the technique for medical reasons, such as donor density and the area to cover, not for pain. See our DHI vs FUE vs Sapphire comparison and the DHI technique page for details.
Does it hurt after the hair transplant?
Expect discomfort, not severe pain. When the anesthesia wears off a few hours after the session, most patients feel tightness and soreness, especially in the donor area at the back of the head.
This is usually strongest on the first night and the next day or two, then eases steadily. Itching usually starts as small scabs form and heal over the first week or so.
Some patients get swelling on the forehead that can reach around the eyes for a few days. It is not usually painful and goes down on its own. Follow your doctor's instructions on how to sleep and move to limit it.
How is pain managed?
- Prescribed medication: your post-op medication kit is included in the package. Take it exactly as the doctor prescribes, and do not add other medicines without asking.
- Rest: plan a quiet first day and avoid effort, bending or heavy lifting early on.
- No smoking or alcohol around the procedure, as both slow healing.
- Hands off: do not scratch or touch the grafts, even when it itches.
- Gentle washing following the routine you are given in the aftercare guide.
How do I sleep after a hair transplant?
The first few nights need some care. The aim is to protect the new grafts and keep swelling down.
- Sleep on your back with your head raised on two pillows or a travel pillow, as your doctor advises.
- Do not lie on the implanted area or rub it against the pillow.
- Put a clean towel or protective cover on the pillow, since some light oozing from the donor area is normal at first.
- Take your prescribed medication before bed if the doctor told you to.
Most patients say the first night is the least comfortable, mainly because of the sleeping position. It gets easier quickly.
When can I go back to work?
Most patients go back to work 5 to 7 days after a hair transplant. Office work is usually fine earlier than physical work. Jobs involving heavy lifting, heat or helmets need more time, so ask your doctor about your case. The day-by-day picture is in our recovery timeline.
When is pain after a hair transplant not normal?
Soreness should get better each day. Contact the clinic if you notice:
- Pain, redness or swelling that gets worse after the first few days instead of better.
- Pus, a bad smell or a fever.
- Bleeding that does not stop with gentle pressure.
- Numbness that does not improve over the following weeks.
Some patients have patches of reduced feeling in the scalp for a few weeks or months. This usually returns gradually as the nerves recover. Patients at Healthy Beauty Clinic are followed for 12 months, so you can send photos on WhatsApp if anything worries you during that time.
Who performs the procedure?
Procedures are led by Dr. Hala Ashkar (also written Dr. Hala Achkar), medical director of Healthy Beauty Clinic, a clinic licensed by the Lebanese Ministry of Health with 20+ years in hair transplantation. In September 2026 she completed a Diploma in Hair & Eyebrow Transplantation from The British Academy of Continuing Medical Education (BACME), a one-year programme at The Doctors Academy in Cairo carrying 400 hours of Category 1 CME credit. She also holds a Diploma in Aesthetic Medicine from the Canadian Board of Aesthetic Medicine (CBAM), awarded in June 2025.

Frequently asked questions
Is a hair transplant painful?
For most patients, a hair transplant is not painful because it is done under local anesthesia. The injections that numb the scalp sting for a few seconds, and after that you feel pressure rather than pain. Mild soreness in the first few days is normal and is managed with prescribed medication.
Are you awake during a hair transplant?
Yes. At Healthy Beauty Clinic in Lebanon the procedure is done under local anesthesia, so you stay awake and can talk, rest and take breaks. If you are very anxious, raise it at the free consultation so the doctor can advise you.
Which hurts more, DHI or FUE?
The difference is small. Both use the same local anesthesia and the same extraction from the donor area, so the back of the head feels the same afterwards. DHI implants grafts directly with a Choi pen without pre-made channels, which some patients find slightly gentler on the recipient area.
How long does pain last after a hair transplant?
Tightness and soreness are usually strongest on the first night and the next day or two, then ease steadily. Itching often follows as scabs form during the first week. Pain that gets worse instead of better should be reported to the clinic.
How should I sleep after a hair transplant?
Sleep on your back with your head raised on pillows for the first few nights, as your doctor advises. Avoid lying on the implanted area or rubbing it on the pillow. A clean towel on the pillow helps, since some light oozing from the donor area is normal at first.
